Service form

To order a product repair, follow these simple steps:

1. Fill out the service worksheet
2. Return the product

Prepare the invoice proving the purchase if you are requesting a repair under warranty and you purchased the product from us. (An invoice is not necessary if you wish to have the product serviced outside of warranty or if you purchased it elsewhere.) If you wish to have multiple products serviced, fill out a separate service worksheet for each one.

After filling out the service worksheet, return the product to our store in the way that is most convenient for you (either in person, by courier, or by mail). Please include the printed service worksheet in the package, or send it to our email address electronically. We will begin servicing the product upon receipt.
